Instructions for completing the Staff Application

You may want to print out these directions. A link to the form and the free Adobe file viewer are below.

The form on the next page is a "fillable" PDF form -- just click in a text box and type your info directly onto the form. Once you've filled out the form, you can print out a copy for submission and one for you personal records.  If you have trouble filling out the form online, or you would rather fill it in offline, save it using the instructions below. You can also save the file and then fill it in later at your convenience. Just double-click on the PDF file and the Adobe file viewer will load, with the staff application in it.

To print out the application:

1. Click the small printer Icon in the upper right corner of the Adobe viewer menu
2. Make any selections necessary for your printer and print it.
