2009-10 Pre-participation Physicals
Physical Exam Procedures
New Athletes will incur a one-time physical fee of $40 (cash or check only, NO credit cards)
A $4 cash fee will be charged to athletes to copy their insurance cards on the day of physicals.
Fall Sports (Soccer, Cross Country, Volleyball)
Pre-participation Physicals (in Heath Building) - August 9, 2009
Fall-Sport Housing Check in Procedures have changed: Please contact appropriate coach for specific time and location for on campus housing check in.
Female athletes at 1:00 p.m.
Male athletes at 2:00 p.m.
Winter/Spring Sports (Swimming, Basketball, Baseball, Softball, Tennis, Track and Field)
Pre-participation Physicals (in Heath Building) - August 29, 2009
Female athletes at 9:00 a.m.
Male athletes at 10:00 a.m.
All athletes will need to stay for a mandatory meeting with the athletic department.
All Returning VU Student-Athletes need to click on the blue link below to download, print, fill out, and mail in the following packet of forms (6 pages) two weeks prior to physicals. (Don't Forget to include a copy of your insurance card - front and back)
Mail completed packet to appropriate VU coach at: VUSC, 55 Fair Dr., Costa Mesa, CA 92626
Returning Athlete Packet
All New VU Student-Athletes need to click on the blue link below to download, print, fill out, and mail the following new student packet of forms (12 pages) two weeks prior to physicals. (Don't Forget to include a copy of your insurance card - front and back)
